How to Make This Form Using eCart 4?

I have this form <http://chicagoblogawards.com/blog/2010/04/07/free-entry-for-chicago-wordpress-meetup-group-members/> posted in a WordPress blog but I can make it as well into an HTML form for use with my website and eCart 4.

My question is how do I go about making it work with PayPal Payments Pro / Express Checkout? I will be adding some prices to form.

Any advice will be extremely helpful and much appreciated.
