Form Builder 1.02 with 'sharp - appointment request'

The time selection should be in a multi select list, this is as designed. I'm including a screen shot of the page that was created for reference. The submit button used in this design is an image, so you can't update the text on it. You could use an image of your own for the submit button, you just need to update the image for the submit button.

When you are applying the form to a page it will use the background colors that you have for the page, on a blank page it should come up with a white background, I'm including a screen shot of what I see in the wizard for reference.

As for the js errors if you are just applying a form to the page you should not choose to remove any of the js code, it should only be there if it is being used by Form Builder. If you modify a form on a page or add another it is possible that you will have extra js on the page that is not used, but leaving it on the page should not cause any problems.
