How to add submitted filename to UE template

I have been trying to accomplish this but the upload hangs and never gets to my thank-you page, so obviously I am doing something wrong. Would anyone be able to give me example code on how to do this?

Can I also suggest that this functionality be added to the extension in a future update, since it seems to be such a fundamental thing to want in a form submission, to know the filename of the file that was uploaded, and to easily be able to return the content of any field that is added on the form. It's difficult on a busy site to know who uploaded which file, particularly when you get 3 or 4 "archive.zip" files in a row.
