Survey Security

I'm creating a survey that I want to only be available to users one time. Right now, a single user could take the survey as many times as they want and jeopardize the results so I need to have a way to keep that from happening. I think the best ways are to have a ISP address check or email field that doesn't allow users to enter the same email. Please let me know what is the best way to make this happen. If I go the email route, I need to make an email form field that checks with the database table attached to the form to see if that email address has already been entered. How do I do that? Thanks for your help...
