Adding a form to a product

Did you add the fields to your db to record these values? If so you can add them to your page with the add to cart on it. In the product details page you need to ensure that you have the new form fields added to the cart form on the page. Once the form fields have been added any values that are entered into them when the add to cart button is pressed will be posted along with the other values for the item.

Once you have got that going you will need to update your cart to add in the new columns. The new columns will be named similar to your form elements. You need these columns to be able to store the info that is entered on the product details page in your cart.

Once you have done all of that you are ready to alter the add to cart server behavior. When you do this you will select the newly added columns and for their value you will select the new form fields you have added into the 'add to cart' form on the page. This is what you need to do to get it adding to the cart. Once you do that you can then edit the confirm pages to ensure you are storing the info in the db.
