Multiple Records Insert issue

Hi...

I'm following your "Inserting Multiple Records" how to document. I have a form with a header row, form elements row and submit button on a php page. I managed to follow the instructions on pages 1-2 to define a repeat selection. However when I move to add the multiple records server behavior by clicking the + sign from the Server Behaviors panel (Data Assist->Insert->Multiple Records Insert) I get the following error from Dreamweaver CS4:

"Your page must contain form elements in a repeating region to use this interface"

I'm not sure what I'm missing from the table...
