Driving Directions...ARG!!!!

I started a thread about this a few months ago. I seemed to have figured this out then, because it worked great for that site, here:

map_directions.php

But today I make a new Google Map and try to set up driving directions, and even with the working version right beside the new one, I cannot figure out what I'm doing wrong. I set it up exactly like the other one, but when I click the lightning bolt to connect the form fields, I don't get the dynamic data window with the form structure, I get the "Setup Instructions", with the "Create a site for this file", "Choose a document type", and so on. I am sooooooooooo frustrated!

If someone could please help me out I promise to tattoo the directions on myself, so I never forget again...
