Using own forms with UE4

I had been using UE3 to send emails using forms that I create myself. These worked fine. I have now upgraded to UE4 and I cannot see anyway that I can use it with the form I have created. I only have the option of a progess bar or creating a form using form builder. I do not want to use FB as I have had no luck so far in getting it to create a suitable form and I don't want to waste another few days trying to learn how to impliment this extension. Is there any way that I can get UE4 to use the form I created or has this option now been removed?

I tried unisntalling and re-installing UE 3 but it came up with error messages so I guess I am stuck with UE4
