I think you are on the rite track with your switch statement. However, rather than have multiple versions of the UE code with different email addresses you should just be setting the email address variable. Then below the switch you can just have the include for the UE. Also, you do not have a default in your statement, you should include this for any value that does not fit into the defined cases. Here is an example of this setup:

switch((isset($_POST["select"]))?$_POST["select"]:"") {

case "AL":
$RecipientEmail = "leifbaker@nj.rr.com";
break;
case "AK":
$RecipientEmail = "leifbaker64@hotmail.com";
break;
case "AZ":
$RecipientEmail = "leifbaker64@yahoo.com";
break;
default:
$RecipientEmail = "<the default email address>";
}
include("WA_Universal_Email/WAUE_forms_1.php");
