On quick inspection, the field names seem to match ok.
which fields aren't updating ?
as some of the fields are updating, i don't think its your database setup, for the fields not updating are you trying to insert text into a numerical field ?
is the date field one of the problems ?, if the format you are trying to input into the db isnt in mySQL format, then it will not work.
if this is the case, then you could process the input string before database input, or if the date is always the current date, you could leave it out altogether and have the database insertthe timestamp automatically.
