Form submits with no validation

Can anyone tell me by looking at the source code what is happening to cause it to stop working? I can't figure out what is different in this file that is causing it to fail completely.  


I have not edited the form in any way. Would something else be causing this? I tried uninstalling and re-installing the extension with the same result.
