Client side validation not working on 2nd form in a page

It seems that the code for the inserts may have been altered slightly, if this is the case it could be the cause for them not showing up in the list.

When it comes to applying the client validations on the problem form you should try manually applying them and not with the wizard to see if you get a different result. You would start by selecting the form and in the Tag panel you would add a new client validation and just build them up one at a time from there. This should eliminate the issues you are experiencing when applying the client validations through the wizard.
