New form-getting dynamic data in fields and other problems

Update

I'm making some headway on this but still have some questions. I think I see now why the dynamic data is in the fields, I'm assuming that is so that it retains the data already entered in case the page has to reload for a field validation error so we're good on that.

Currently, I'm able to get it to insert the data but the only way I can get it to work is if I rename the fields after I've "finished" the form. That's particularly true on the checkbox groups where the fields are all named with the same group label. Is there any problem with doing it that way?

Thanks,
