Photo won't upload

Hi,

I am using Digital File Pro with my Insert page from DataAssist, but when I add things to the Insert page, it never saves the image that I select to PHPMyAdmin. When the detail page comes up, all the other information is there, but not the photo. What could I be doing wrong? Any ideas?

Attached is a screenshot of my Insert page if it helps.
