Map/Directions Do Not Show When Sending Form Data To It

I have a page the displays a Pro Map with the hard coded destination address entered at design-time and it works fine. On that same page I also have an address form that once submitted opens a new page with another pro map to show driving directions from the form address.

I've tested that the form is successfully submitting the data but I just get a blank map placeholder and nothing else. Below is the page with the form for testing.

I am using Dreamweaver CS4 and the 1.0.4 version of the extension.
