found problem

the line:
<?php require_once("WA_DataAssist/WA_AppBuilder_PHP.php"); ?>
was missing. Don't know why, maybe I accidentally deleted it? So I added it back in and presto, the info all entered into the database.

So next I'll add two more fields and hope it goes ok.
