1.0.2 not redirecting or sending

Hi, I read through all the posts I could find that hinted at a similar problem, including one of mine from long ago, but I'm still stuck. Apologies if I missed one with an answer... I also set up a ticket with the host but I feel you all can help better.

Page in question is here contactgch.php

Globals is currently at standard re email server, i.e. blank.

I'm getting these warnings (need to view source to see them as they get covered up):

I did find one whitespace space and removed it but it didn't change anything.

There was a thread with these two lines mentioned but the dude just said the host fixed it...

The validation works well or seems to.

Can you see anything my still noobie brain is missing? Should I wait to see what the host says?
The site is on accuwebhosting.com on a Windows starter package that is supposed to support PHP 4 and 5 if that helps.
