Problems with Include Attachments with your email form

I'm trying to follow the Add file upload field directions in this WA guide, p.1 and 2. I don't find any "Label" field in the box that comes up after clicking on Forms | File Field option. I also don't see a "Style" button with Wrap with label tag command. Finally, when I go to the next section to apply digital file pro upload behavior, I can't find the 'attachment' file field I created in the previous section. Why would the instructions here look so different than what I actually see in DW? With little experience with this kind of feature, it is causing me great fits because I don't know how to troubleshoot. Does the article need to be updated for CS4?
