I need to add more selection fields to form

Many thanks Eric. I have made the modifications that I wanted to make.

There is this other problem -- when is ljcontact100109.php is launched, all the three slection options came up checked. I will like them to come loaded unchecked.

Q: Is there somewhere on the code where this can be done.

--- I made the changes for value="1" to value="0" for the check boxes, but the boxes are still coming up checked when the page is loaded.
