In case anyone is following this thread, I found a workaround:

After I click the "+" in Bindings tab in the Server Behaviors panel, I selected "Form Variable" instead of "Form Data" from the pop-up list, at which time I was prompted to enter a form variable name. This created a new grouping in Bindings called Form with the identified variable listed in that grouping. I then repeated this step for each variable in the form that I wanted to put in the email message.

After that, I then inserted each of these variables to the appropriate location in my email message. Rather than seeing the PHP icon that I normally would, I get information like {Form.variablename}. But this seem to have worked!
