A bunch of the bindings are in the wrong syntax. Most likely you did some hand edits and Dreamweaver didn't recognize them properly when you edited with the UI and it messed it up.

You can easily correct the issue in the code on lines 78,79,80,83 and 88

The lines have code like:
$InsertQuery->bindColumn("NeworUsed", "s", "\".($_POST['NeworUsed']) .\"", "WA_BLANK");

but it should be:
$InsertQuery->bindColumn("NeworUsed", "s", "".($_POST['NeworUsed']) ."", "WA_BLANK");

One important thing is to use a double space after the $_POST[] value before the double quotes. That is probably what was missing when you hand edited and that is maybe why it got mangled when you edited.

You can also go into the UI and re-set those values from the bindings tab to ensure proper syntax. That might be the better option to prevent any other minor typos.
