I'd have to reproduce it. Usually that is a redirect on a page that has empty lines before the redirect that prevent the redirect from happening. It could be another error above a redirect that causes the header error. The header error is almost always a failed redirect due to content already being written to the page.
The full error should include page names and line numbers. If I could see that, and see the pages referenced, then I could fully diagnose it.
