Warning: session_start()........Cannot send session cookie - headers already sent by

I had this form for years on their site and they updated the design. I took the old php file and just changed the code for the header and footer, all the rest of the file stayed the same. However it is now throwing errors. I looked in the two files its complaining about but don't see anything wrong. It seems to NOT be emailing the form and it never gets to the redirect page. I used Web assist extensions to build all this years ago. Can you assist? Form is posted at: http://hemogenix.com/contact_us.php. Attached the error page and the two files associated with errors. THANKS!!
