Your FROM address is set to come from the form... and your TO addresses are GMAIL and AOL addresses.
Gmail and AOL have one of the most strict anti-spam policies. They can certainly detect that the emails are coming FROM your website and not from the email address they claim. That is probably the primary issue. To fix that, just update the FROM address to a valid email address using the same domain as the website. You can still set the reply-to address to use the email address from the web form so that you can reply to the emails sent without hurting the deliver-ability.
Another issue could be the DKIM or SPF record setting on the server itself. I use: https://www.mail-tester.com/ to test my emails and refine the server settings and email content to improve spam scores to help make sure they get delivered when I have issues.
Lastly you could update to Data Bridge v2. We have an improved email code base that can help with delivery including the ability to send with authenticated SMTP like you would with your phone or computer, which have much better SPAM scores with less configuration requirements.
