PHP dynamic forms won't work on localhost but do on remote server

I have several PHP forms that work perfectly on my remote host server but will not work on localhost. The MySQL server is operational on localhost so I'm at a loss as to why the forms work remotely but not locally. Any suggestions where/what/how I should look to solve this? I am attaching one of the forms (incl_admin_form_edit.php) for your review.

UPDATE
Thanks for your very prompt response. I found the issue to be the client typo'd his last name and the record was not being found when searched for. After correcting the spelling, the form worked. So this has been resolved.
