MySQLi Insert process not working

Please attach an actual copy of the page instead of copy and pasting the code when you want me to look at a file. It helps when I can open it with Dreamweaver.

I'd probably have to debug this on your server. My best guess is that it is a conflict with the other script on your page. I see you have some repeated jquery form validation code that might be causing issues. Can I get FTP access and a url with steps to reproduce?
