Search options working properly

Hi.
I am having an issue with the medical database site that you have helped me with in the past, multiple times. I do not know if this is new or if I just missed it.
The search page is at 4patientsafety.org/mdb/search.php

What we noticed is that when I select a Gender from the menu and another multi-option drop down it is searching 'OR' instead of 'AND'
It seems to be fine and select the correct gender if I select it and one of the items that is NOT a multiple choice array

So if you search Female from the gender dropdown and then say OBGYN from the Specialties menu it brings up all OBGYN doctors not just female.
