Bootstrap-select not working in a framework plug-in page

Hello Ray,
I created a new page with DB2 and bootstrap 3. I applied the framework plug-in to recreate the dependent dropdown menu scenario.
It almost work.
All the SELECTs in the page use a bootstrap plug-in (bootstrap-select) and they all work except the one contained in the framework plug-in.
The problem is that the SELECT in the plug-in page doesn't trigger the bootstrap-select plug-in as the other ones.
If you trigger the Provincia menu you see the dropdown is working (you can even search in it) and it is style correctly.
But the SELECT in the plug-in page seems to not get the same style and operational mode.
Could this be related to the framework plug-in?
In general, what have I to put in the plug-in page (I'm talking about external scripts, plug-ins....).
Or perhaps, have I to remove all content from head and leave only the body code (along with the php code at the beginning)?

This problem, probably, can be related to any kind of jquery/bootstrap plug-in, actually.

This is not clear to me, at all.

You can see the credentials and the page as private message.
TIA
