PHP forms posting to database not working

Two forms that you guys helped me develop a few years ago have stopped working. I tried to fill out your premium support page but it wiped out on me twice. See the attached two files. These files help me manage a membership database for active and lapsed members. Once a member becomes delinquent in their dues, we move them over to the lapsed member table in the database. In the Active Member form, this was accomplished by a radio button (yes, no) - hitting No moved the data over into the lapsed table and populated the Lapsed Member form. Now, when I hit No, the member data is just disappearing: it is not reappearing in the lapsed member table and the lapsed form is empty. Somewhere along the line it is losing the userID connection I think.

I need immediate help with this and will pay for a session if need be. I have 100 people to process.
