This Contact Us is the last page of an 8 month project and I can not get it to work. Can you give me a few suggestions on how to fix?

I am trying to used the Name filed for the From, CACCA.CAPS@gmail.com for the To, the Email box for the CC and my address for the BCC. Upon send, I want the screen is to return to caps_home.php. After submitting, the contact_us.php screen re-appears blank and no email is sent.

I have looked at all the tutorials and cannot figure out what is wrong.
