Validation for Captcha

I dont quite understand what is being said.

I have had a look at the code and I cannot see where there are 2 Server validations.

What is the "CAPTCHA validation"

I notice now that data from the selection box is not being placed in the database. I am almost sure it was working last night.
Later:
The selection box does not seem to work at all.
I have tried it dynamic and as a straight list but is is not entering the database field.
