That file should have been included with the installation. Please try reinstalling the extension. After installation, open up a page in Dreamweaver and let the secondary installer complete. Then restart Dreamweaver one more time.

If the issue continues I'll have to do a screen sharing session in order to debug this with you directly. You can reply with a US phone number or Skype contact information and I'll contact you to schedule a time.
