Checkboxes on form don't work after converting to Bootstrap classes

I was having difficulty adjusting the WA forms to work in a responsive layout, so I reconfigured some of the WA classes to Bootstrap. This included some of the list tags, but didn't mess with anything I thought would affect the form, such as Names or IDs. Some of the php was also relocated into new rows.

When uploading the page, I also chose to re-upload all the related files. Now, the form's two checkboxes don't return any results, though the search field still works properly. Don't know if the problem is with the classes, tags, php relocation, or if working on the page somehow adjusted one of the WA include files.
