A few things to make sure of before our debug session:
1) Are you able to create a database connection and successfully test it?
2) Are you able to create a Recordset on the page?
The extension should be popping up with an error message "You must add a recordset to the page before applying Database Search". It shouldn't work on a page without a recordset, so the issue is that the error message isn't displaying.
Please make sure you have a working database connection and recordset on the page and if it still doesn't work post back with your contact information and I'll do screen sharing with you to debug why.
