Javascript errors trying create MySQLi recordset simple mode

Having had this extension for a considerable time now but not really got round to using it properly, I'm having problems getting it to work. It will add a recordset in advanced mode OK but throws up js issues when switching to simple mode. I've uninstalled and reinstalled the extension (more than once) to see if that would work but still no good.

While executing onClick in MySQLiServerBeh-PHP-AdvRS.htm, the following JavaScript error(s) occurred:
At line 338 of file "F:\Program Files\Adobe\Adobe Dreamweaver CS4\Configuration\Shared\MM\Scripts\MySQLiRecordsetDialogClass.js":
Cannot find file F:\Program Files\Adobe\Adobe Dreamweaver CS4\Configuration\Commands\ServerBeh-ASPNET-SimpRS.htm

At line 338 of file "F:\Program Files\Adobe\Adobe Dreamweaver CS4\Configuration\Shared\MM\Scripts\MySQLiRecordsetDialogClass.js":

(Using DW CS4 on Windows 7 Pro)

Having searched to see where those files all actually are has left me even more confused as to what should be where!
