How to add Universal Email 'blank' template page bindings; form page loads slow.

Back to the earlier post, I've named everything in the Contact Us page, but still can't get the form items to show up in the Blank.php bindings.

On the Contact Us page, there weren't any dynamic behaviors for the radio button or checkboxes, so I tried to create manually. The names of the checkbox items appear in the 'Checkbox:' area, but not sure where to go from there. The 'Check if:' area wants me to select something from the form Persistent Form list, but none of the checkbox items are listed.

Also opened the 'Dynamic Attribute' behavior, but neither radio or checkbox items are listed.
