Email formating

I have a simple request info form on my site. the email that is generated back to me seems to have code mixed in with the text. and not all of the text fields in the form show up in the email such as the E mail field. It is a simple form see below:

Name:

E Mail:

Town:

Sport:

Team Name:

Date:

Time:

Location:

Address:


This is what is a sample of the email that is generated:
