Cannot protect a page by MySQLi authentication. Restricted page opens directly - does not open logon page first.

Cannot protect a page by MySQLi authentication. Restricted page opens directly - does not open logon page first.

[OSX 10.11.3, Dreamweaver CS6, Macbook Pro 15" Retina laptop, MySQLi 1.0.4]

I've added a MySQLi restrict access behavior to a php webpage but I am able to open it without via a link without it opening the logon page first. I'm attaching the logon and restricted pages for you to review. Please let me know what I'm doing wrong so these two pages work together properly.
