Select all styled checkboxes - premium support

This is something I've been looking at and very nearly got working. Basically some forms with nicely styled checkboxes, and also an option to select all the checkboxes.

I have the styling working with jquery, and I have the select all option working. Just not both at the same time.

The closest I have got has the check / uncheck all toggle out of sync - so when then 'select all' box is checked, the rest are unchecked, and vice versa.
