form validation not working

I am having a problem with form validation in eCart 6. When failing to fill in one or more fields on the order form, then subsequently being prompted to fill them in, ONE of the fields that was filled out is lost every time, plus the date chosen via date-picker is also lost. I believe we fixed this before (but couldn't find anything to refer to in the thread), and I don't think I have done anything to change it, nevertheless it is not working. Attaching the page below. The form field in question is Left logo, and also date picker.
