couple of problems i see:
1) I am not able to make a telnet connection to the SMTP server address and port you provide. Double check that these are correct. For a valid SMTP server / port, I should be able to make a telnet connection to
2) You are using the dynamic email form element for the from address. Many hosts will require the from address be set to a valid email address for our domain. using the email address entered in the form will trigger the SMTP server to treat the email as pam.
In the UE server behavior, click the From button to access advanced from settings. you can use the static email address for your domain as the From address, but then bind the Reply To address setting to the dynamic form element.
one way to help trouble shoot email issues, is to not set the go to address in UE so it does not forward to another page, then add code to dump the session contents in the body of the page:

the session contents will contain the status of passing the email to the SMTP server. IF that returns success and you are still not getting the email, then the SMTP server is not replying the email for some reason, and you will need to contact the host to investigate further.
