Emails stopped going through

Hi Jason,
I changed something on the contact us form I was using. The emails go through to the main person but not to the person listed on the form (email address entered on the form). I know on the block template i changed the name on the form elements so they would go through as a proper title name but that should affect the to email address should it? I keep thinking it must have something to do with matching up the field names bitu I can't make it work.

to the CC entry ...

It has to look at the ID correct?



Just to update this as I test the email form and send a test email rather than going to the next page i get the for show that the fields were not filled out properly (see contact_us.jpg). It is supposed to go to an "email_sent.php" page.

Sorry but I'm stumped again. Could you please have a look at the attached and let me know if anything stands out?
