Undefined index: /PHP_SELF

Good day.

Every time I press checkout onn my confirm.php page I get this message in my address bar

 Notice: Undefined index: ../PHP_SELF in C:\xampp\htdocs\henkorsite\confirm.php on line 886 

I attached a rar file with my confirm.php page, let me know if you find any thing or if you need any thing els.
