Transaction Not Completed error

I set up a Cafe Commerce site for a client, and she received an email from a customer saying he gets this error at checkout:

Transaction Not Completed Your transaction could not be completed. There is a problem with the credit card or billing information you have entered. Please, return to the checkout page and try again.

He has tried several times and says his information is correct. I tried putting the store in test mode to do a test order and that worked fine.

Is there any other reason he might be getting this error other than user error?
