Chackout page not loading - blank

Hi Jason, Apologies for troubling you again. But I am stuck. I have added a database to record orders and reformatted the phone number field using the Jdquery validation tool. I have added the database as I had a sale this morning where the order details were sent through to Paypal and recorded but no name address email or telephone number. The database is set up on my server correctly from the checkout wizard. But unfortunately when I tried to test everything the checkout page does not display. You have my FTP details in the private messages above. I have also attached the current checkout page. I hope you can help. Best wishes Robert
