Uploading files not working

With Ray's help, through a private session I created a file upload page that seemed to function on the test server but now that I've uploaded it to the live server it no longer functions. Ray has suggested that I upload the files to root but that died not work and contacting the ISP they said that I could not get to root that way. I tried then to change it to upload the files to public_html but I have been unable to make it work in any fashion. The directory I want the files to go into is artists_files. I now have three of these directories created in different location and have tried .../.../artist_files
.../artist_files and just artist_files but the uploads do not go anywhere. I have attached the page. I have tried to create a new upload page but still cannot make it work. Can you point me in the right direction?
