JavaScript errors with CSS Form Builder

I'm trying to CSS Form Builder to make a page where a user tells uses an HTML Form Editor to tell their story and send in a photo. I want the page to send an email and to enter the info into a database. However, it constantly errors out. I even tried starting from a brand new site and it doesn't work. I use Mac 10.7.5, Dreamweaver CS5 and CSS Form Builder 2.0.1. The error I get is: While executing applyServerBehavior in CreateEmailMessage_WA.htm, a JavaScript error occurred. After this I will have JavaScript errors that I can get rid of by deleting my cache and restarting Dreamweaver. However, every time I try to start over or fix what is there I get stuck in the same error loop.

I've attached the files that are created when I try to start from a fresh pages.

Any help is greatly appreciated.
