Troubleshooting BUILDING A MULTIPLE ITEM ADD TO CART INTERFACE

My client want a page for their recommended starter package that their franchisees can order with a single click. However, they want to be able to customize the package, so we can't just do a single product as the package. I've walked through the archived tutorial and everything seems in order, but when I click add to cart, I'm not forwarded to my cart page, and the cart is empty. I'm unsure how to troubleshoot as I'm not receiving any kind of error messages.

I'm attaching the page. Any incite would be appreciated.

Also, I want to add quantities into the page as well (hence the quantity field), so the client can set suggested quantities, but I haven't gotten that far yet as I haven't been able to get the basic multiple add to work yet.
