From values not showing in email or in bindings **FIXED*

I have added Universal Email behaviour to an existing contact form, but the email that comes through contains no information from the form. Also, when I was going through the steps in the Wizard to create the email no form fields were showing in the bindings panel.

I checked the form method was selected, I checked the enctype was set and I checked that the form fields were named properly. I even cleared the cache in Dreamweaver. In the end I had to create a new form on a blank php page in order to get the fields to show in the bindings panel. I then copied the PHP code generated by Universal email into the original page where the proper form is located.

I'm confused as to why it works on one form but not the other? I've zipped up the files and attached them. Any help is appreciated.

***EDIT***
I fixed the issue. It seemed that even though I had filled in the name in the properties inspector in Dreamweaver, this hadn't actually updated the code. I manually added in the names for each of the fields and everything works now.
