Duplicate entry '127' for key 'PRIMARY'

I know I had ran into this on one other project and thought I had found the answer from a support thread, but haven't been able to find it again. I'm duplicating code from project to project so it's likely the same issue. On my insert page, inserting new records triggers the autoincrement on the DB table correctly all the way up to record 127. At that point, it starts kicking "Duplicate entry '127' for key 'PRIMARY'", as if the autoincrement suddenly stops working at 127. If I delete record 127, it inserts the record fine, but the next entry then kicks the same error.

I remember thinking that it had to be something with the host server, but in the end I think it was a code issue. I just can't for the life of me recall what the problem was. Any insight? Insert page attached...
